Tiling Inside Corners and Ceiling Edge

Inside Corners: Are inside corners as simple as a butt joint spaced by 1/8" where the back wall is set first, then the side walls butt up to the back wall like this picture?


Or, would you do something like this (which seems excessive albeit fancy)?





Ceilings: Do we typically butt the tile up against the ceiling and caulk the joint, or do we leave a 1/8" gap and caulk the joint? My story pole wants to know. ;).


Either way, I'm correct in assuming that any change of plane on an inside corner that doesn't have trim should be caulked, not grouted. Yes?

We use butt joints unless it is a tile with relief on it (like those wavy tiles). Here in our area we go against TCNA recommendations and use hard grout in the changes of plane, because it looks better and the availability of color match silicone is not good here. If there is constant movement and we get a call back (rarely) then we go back and use silicone.
