Grout Discoloring and Coming Out of Shower Pan

Hello All!

I'm new to the forum, but have been a tile contractor for about 5 years and a long time fan of the Tile Coach Youtube channel.

I did a shower back in August of this year: used a Schluter foam fan, fashioned large format tile for the floor with relief cuts, and grouted the entire shower with mapei FA ultracolor grout. The client reached out to me today telling me that the grout in the shower floor has started to discolor (turning a reddish color) and develop holes. This issue is not present at all in the shower walls.

Wondering if any of you have any idea what is going on and how to remediate it? I obviously could just clean the grout, fill the holes, apply some Mapei grout refresh and move on, but I don't want to just do that if this is indicative of a larger problem. This is what the shower pan looked like after I finished:

This is what the shower pan looks like now:

Any guidance would be greatly appreciated! Thanks in Advance.

Thanks Ben! I really appreciate your feedback. I thought the holes were just do light grout fill hole in some of the seams, leading to air pockets underneath, which after some use are eventually wearing through. It was more the red that made me reconsider that something else might be at play. But that's interesting about the bar soap. I'll definitely ask them. Yeah, I can't imagine there is  (or at least I really really hope there is not) an issue with the pan as I used Schluter corners and seam tape, as well as 2-3 coats of Mapei aquadefense, and flood tested it over a full weekend. No issues with the pan came up before. But I'll do whatever it takes to fully resolve the issue. Really hoping it's just cleaning and grout filling!
